**Please note: This form must be completed in its entirety.  All requests for funding must be given to the chairperson of the Endowment Fund Committee no later than two weeks prior to a regularly scheduled meeting of the Endowment Fund Committee.  Meetings are held the second Tuesday of February, May, August and November.
